Enrollment Forms

There are certain forms that are required upon registration, all of which are available in writable format online at durangoschools.org or in paper at all school sites. Please note that the census form and home language survey are both captured during online registration.
 
Below is a summary of each form, so that parents and guardians can download the forms they need to return to their school.

Health Information Form – This form is mandatory for ALL students.

It allows your student to access the school nurse and receive medical services at the school. This form must be completed and signed each year by the parent. The form also authorizes 9-R to transport students via ambulance in the event of an emergency.
 
Please complete this form and turn it in to your child's school.
 

Home Language Survey

The Office of Civil Rights and the Colorado Department of Education require school districts to determine the dominant language spoken by your child to help provide meaningful instructional programs and appropriate communications for home. This information is necessary to provide the most appropriate placement and instruction for your child and will not be used for any other purposes. 
 

Military Opt-Out Form

The federal Every Student Succeeds Act requires the Department of Education (DOE) to provide to military recruiters, upon their request, the name, address, and telephone number (including unlisted number) of secondary school students. If any secondary student or the parent/guardian of a secondary student does not want the DOE to provide the requested information to military recruiters, the secondary student or the parent/guardian must “opt out” of providing such information. 
 

Title VI Student Eligibility Certification

In order to apply for a formula grant under the Indian Education Program, your child’s school must determine the number of Indian children enrolled. Any child who meets the following definition may be counted for this purpose. You are not required to complete or submit this form to the school. However, if you choose not to submit a form, the school cannot count your child for funding under the program. This form can be found on our Title VI page.
 
 

School-Based Health Center Opt-In Form

The Durango SBHC is located at Durango High School, all 9-R students are eligible for services at this health center. In partnership with Pediatric Partners of the Southwest, the center offers a practical and affordable approach for preventive and primary health care services to students during school hours. Notification of Inadequate or Temporary Housing as defined by the McKinney-Vento Act:

 
The McKinney-Vento Homeless Assistance Act ensures the educational rights and protections for children and youth experiencing homelessness. Please complete the form below if your family is currently experiencing homelessness and turn the form in to your child's school. The following forms are available at your school's office:

FERPA Opt-Out Form – The Family Educational Rights and Privacy Act (FERPA) provides families options on how they want to protect their student’s privacy. Students are automatically opted-in unless parents notify the district of their preferences by filing an Opt-Out Form. Unless opted-out, FERPA allows schools to publish student information in directories, including yearbooks, team rosters, honor roll etc. 
Media Opt-Out Form – The school district utilizes photos and videos of our students, staff and parents for both internal and external communications pieces, including but not limited to videos, brochures, emails, websites and newspaper articles. There is no better way to demonstrate all the great project-based learning, innovative programs and distinguishing opportunities our schools offer than through communication pieces that capture the story through pictures. If you do not wish to give permission to have your child’s photo taken or filmed, please make sure you fill out a Media Opt-Out Form, available in your school office.
