Child Find

Child Find is committed to supporting families through the process of identification, evaluation, and continued appropriate services for children with possible delays and or disabilities. The Child Find team evaluates children who are ages 3-5 and also coordinates the identification and evaluation of school-aged children in private schools.
 
The Child Find team offers support and developmental screenings in the areas of speech/language, fine motor, and gross motor skills, learning, hearing/vision, and social and emotional adaptability at no cost to parents. To access screening or make a referral for a child 3-5 please complete the Preschool Child Find Referral.
 
If your child is a school-aged (kindergarten-12th grade) homeschool or private school student and lives or attends school within the Durango 9R school district boundaries please complete the School-Aged Child Find Referral.
 

If your child is birth-3 years of age, contact Community Connections at (970) 565-8389. Community Connections supports kids and families in La Plata County through Early Intervention programs. Community Connections holds evaluations and children receive an Individualized Family Service Plan (IFSP), with services generally received in the home settings.
