Benefit Application Forms

While all meals are free this school year, there is more to applying for meal benefits than just school lunch!
Although all students will receive meals for free this school year, it’s still important for households to provide income information when requested. Gathering this information allows Durango School District to receive state and federal funding we qualify for. These funds go directly to schools to help cover the cost of meals and support after-school activities and other nutritional programs for students. Plus, qualifying households may be eligible to receive Summer EBT benefits, receive discounted school fees, class materials, bus passes, utilities support and more.
Anyone that would like to receive benefits should fill out the online form in the Infinite Campus Parent Portal. These benefits could include discounts on trolley passes, yearbooks/sports equipment, and Durango Recreation Center. These forms are based on income and household size, and there is no guarantee of eligibility. Please visit this link if you would like to see the USDA Income Guidelines before you apply. If you would prefer to print a form and submit by mail/in person, please select a form below. Please note this will take additional processing time, and it may be up to two weeks before you receive notice of benefits in the mail.
NOTE: If you were directly certified in the past due to SNAP or Medicaid eligibility, there is a strong chance that your household will be eligible again. Please contact the Food and Nutrition Services office if you are unsure of your current eligibility before applying.
