Records & Transcripts

If you have any questions regarding Student Records or Transcripts, please call Sharon Matheson in the Student Records Office at 970-259-1630, x2311 or through email at smatheson@durango.k12.co.us. If you would like to submit a Records/Transcript Release Form (found at the bottom of this page) you may fax it to 970-375-3997 or email it to: smatheson@durango.k12.co.us

If a former student wishes to have a copy of his/her official high school transcript sent to a college, university, or other institution, a Records/Transcript Release Form (found at the bottom of this page) must be sent to Durango School District 9-R Student Records.

Due to federal law, information concerning a former student's record cannot be released until a records release form has been submitted. If a student is under 18 years of age, the form must be signed by a parent/guardian. The request must include the correct mailing ad­dress to which the transcript is to be sent.

Can I get a copy of my child’s immunization records?

How do I have my child's immunization records sent to a new school or college?
Use the Records/Transcript Release Form at the bottom of this page. Instructions are provided at the bottom of the form or have the new school request them for you.

How do I withdraw my child from a Durango School District 9-R school?
Go to the 9-R school where the student is currently enrolled.  Sign Withdrawal Forms so your child’s records can be processed for the new school. You must provide the new school's name and address.

I have moved, but did not complete the withdrawal forms for my child.  How do I have my child's records sent to the new school?
Use the Records/Transcript Release Form at the bottom of this page. Instructions are provided at the bottom of the form. You can also sign a Records Request Form at the new school.  The new school will fax the Records Request Form to Durango School District 9-R's Student Support Services office at 970-382-0588 for processing.
